How to sign up for COVID-19 vaccination in Metro Manila's 17 LGUs

Amid the rising cases of the coronavirus disease 2019 (COVID-19) in the country, many are wondering if and when they can get access to a vaccine.

Currently, the Philippines has two vaccines: AstraZeneca and Sinovac. Local government units have started inoculating residents based on priority status, with medical frontliners (A1), senior citizens (A2), and persons with comorbidities (A3) at the top of the list.

According to the Department of Health, the A3 group includes adults aged 18 to 59 with “any controlled comorbidity,” but priority will be given to those with following:

1. Chronic respiratory disease
2. Hypertension
3. Cardiovascular disease
4. Chronic kidney disease
5. Malignancy (cancer)
6. Diabetes
7. Obesity
8. Cerebrovascular disease
9. Chronic liver disease
10. Neurologic disease
11. Immunodeficiency state

Due to the vaccine shortage, however, only patients with the first seven listed comorbidities will be prioritized for now as they are at highest risk of hospitalization and death.

Those in the A3 group must show any of the following documents as proof of comorbidity, with some LGUs preferring the first two documents:

• Medical certificate issued within the last 18 months
• Medical prescription issued within the last six months
• Hospital records such as discharge summary and medical abstract
• Surgical records and pathology reports

Meanwhile, some persons with comorbidities need to provide a medical clearance from their doctor before they can get vaccinated. These are patients with the following:

• Autoimmune disease
• HIV
• Malignancy (cancer)
• Transplant patients
• Undergoing steroid treatment
• Poor prognosis or bed-ridden

If you belong to the priority groups, you can sign up for vaccination with your LGU. 

Regardless of your city, it would be wise to bring supporting documents with you so you can be vaccinated without delay. In summary, here is what you need:

• Valid ID with address
• Proof you belong to a priority group
  • A1: PRC ID, company ID, certificate of employment
  • A2: Senior citizen ID, NSO birth certificate, valid ID with birthdate
  • A3: Proof of comorbidity and medical clearance
• Proof of vaccine appointment or pre-registration
• Ballpen, face mask, and face shield

For Metro Manila residents, below is a city-by-city guide on what you need to do in order to get your shot.

Caloocan City

• Fill out the profiling form. Access it here.
• Prepare the following requirements:
  • PhilHealth ID (if available)
  • Valid ID (Senior Citizen ID, PWD ID, PRC ID, other IDs)
  • Proof of comorbidity
• Note: Walk-ins are accepted depending on availability of slots, but those who underwent profiling will be given priority.

Las Piñas City

• Fill out the profiling form. Access it here.
• Attach a scanned copy or photo of valid ID or proof of residence.
• Manual registration is also an option. Each family will receive profiling forms, which can be submitted to the barangay or the homeowners association.
• Attach a 2x2 ID photo and any government-issued ID with address.
• Prepare the following requirements:
  • Proof of comorbidity
• Wait for your vaccination schedule, which will be sent via text message.
• Note: Walk-ins are not accepted.

Makati City

• Fill out the profiling form. Access it here.
• Prepare the following requirements:
  • Government-issued ID
  • Proof of comorbidity
• Take note of the confirmation number and wait for the confirmation text for your vaccination schedule.
• Note: Walk-ins are not accepted.

Malabon City

• Fill out the profiling form. Access it here.
• Registration via the barangay health center is also an option.
• Prepare the following requirements:

  • Medical clearance
• Wait for the confirmatory text message for your vaccination schedule.

Mandaluyong City

Manila

Muntinlupa City

• Wait for the email to get your schedule and QR code.
• Prepare the following requirements:
  • Valid ID with address and signature
  • Proof of comorbidity
• Note: If more than one week has passed and you still haven’t received your QR code, you may drop by as a walk-in to the vaccination site assigned to your barangay.

Navotas City

• Fill out the profiling form. Access it here.
• Registration via the barangay health center is also an option.
• Wait for the confirmatory text message for your schedule.
• Bring your QR code on the day of vaccination.

Parañaque City

• Fill out the profiling form. Access it here.
• Prepare the following requirements:
  • Healthcare workers: PRC ID, Company ID, PhilHealth ID, Certificate of Employment showing your are working in a health facility within Paranaque City
  • Senior citizens: Senior citizen ID, medical clearance or certificate
  • Persons with Comorbidities: Proof of comorbidity
• Wait for the communication by the City Health Office via text message, call, or email for your vaccination schedule.

Pasay City

• Pre-registration can also be done by contacting the barangay.
• Prepare the following requirements:
  • Valid ID
  • Proof of comorbidity
  • Medical clearance
• Call your health center for the schedule.

Pasig City

• Fill out the appropriate profiling form:
  • For Senior Citizens.
  • For Persons with Disabilities.
  • For Persons with Comorbidities.
  • For Private Healthcare Workers.
• LGU also conducts house to house profiling.
• Wait for the text advisory for the vaccination schedule.
• Bring the following: Valid ID, PasigPass QR code, medical clearance for persons with comorbidities

Pateros
