Pinoys in Italy advised to submit self-declaration form, says Philippine Embassy
The Philippine Embassy in Rome has advised Filipinos to submit a government form that would have to be countersigned by a police official include details of their activities that include shuttling to work, medical concerns, other basic necessities.
In an announcement posted on social media, the embassy said the Italian Ministry of Interior has issued a revised Autodichiarazione or Self-Declaration form "for movements allowed by the Italian authorities."
"The new form includes a declaration that the person signing is not under quarantine and is not positive of COVID-19," the embassy said.
The post, dated March 17, 2020, was issued as the Italian government continue to struggle in controlling the spread of the coronavirus disease 2029 or COVID-19.
As of posting, Italy has recorded the most number of deaths in one day for any nation at 475 as reported by Reuters.
In filling out the new document, the individual is require to ask the police to also sign the self-declaration. This is a form of certification that the document was made the presence of the official and that the person has indeed presented a valid identification document.
The individual does not have to attach a photocopy of an identification document to the self-declaration form.
In this case, the person no longer needs to attach a photocopy of his/her identification document to the self-declaration.
Filipinos are reminded to declare accurate information as police authorities could verify the accuracy of the information provided.
Otherwise, non-compliance with these regulations may result to a penalty of Euro 206 or 3 months imprisonment.
